Clinical value of serum VEGF and VEGFR-2 detection in evaluation the efficacy of neoadjuvant chemotherapy for rectal cancer

摘要: [目的]探讨血清VEGF与VEGFR-2检测对评估直肠癌新辅助化疗疗效的临床价值。[方法]选择2013年7月~2014年7月医院收治的55例直肠癌患者作为研究组,患者采用奥沙利铂、亚叶酸钙和氟尿嘧啶组成的FOLFOX6方案,患者均完成2个疗程的治疗;另选择同期在医院体检的50例健康人群作为对照组,采用酶联免疫吸附试验检测两组血清VEGF、VEGFR-2水平表达水平。[结果]中分化、高分化组血清VEGF、VEGFR-2水平显著低于低分化组,高分化组血清VEGF、VEGFR-2水平又显著低于中分化组(P<0.05);Ⅳ期直肠癌患者血清VEGF、VEGFR-2水平显著高于Ⅲ期(P<0.05)。化疗前研究组血清VEGF、VEGFR-2水平均显著高于对照组,化疗后CR+PR组、SD组血清VEGF、VEGFR-2水平显著降低,PD组血清VEGF、VEGFR-2水平显著升高,化疗前后比较差异具有统计学意义(P<0.05);其中化疗后CR+PR组、SD组血清VEGF、VEGFR-2显著低于PD组,CR+PR组血清VEGF、VEGFR-2又显著低于SD组(P<0.05)。相关性分析显示研究组化疗前后血清VEGF、VEGFR-2表达均呈正相关关系(P<0.05)。[结论]VEGF、VEGFR-2是直肠癌发生、发展的敏感性生物学指标,通过检测患者血清VEGF、VEGFR-2水平能够有助于评价辅助化疗疗效及预后。Abstract: [Objective] To explore the clinical value of serum VEGF and VEGFR-2 detection in evaluating the efficacy of neoadjuvant chemotherapy for rectal cancer.[Methods] A total of 55 cases of rectal cancer patients treated in hospital from July 2013 to July 2014 were selected as study group,constituted by oxaliplatin,leucovorin and fluorouracil of FOLFOX6 therapeutic regimen were treated for patients.All patients received 2 courses of treatment.Another 50 healthy people were selected in same period as the control group,and the levels of serum VEGF and VEGFR-2 expression were detected by enzyme-linked immunosorbent assay in two group.[Results] The levels of serum VEGF and VEGFR-2 in medium differentiation group and highly differentiation group were significantly lower than those in the low differentiation group,and the levels of serum VEGFR-2 and VEGF in highly differentiation group were significantly lower than medium differentiation group(P<0.05).The levels of serum VEGF and VEGFR-2 in Ⅳ stage were obviously higher than those in III stage(P<0.05).Before chemotherapy the levels of serum VEGF and VEGFR-2 in study group were significantly higher than the control group,after treatment the serum levels of VEGF and VEGFR-2 in CR+PR group and SD group were significantly lower,PD group were significantly increased,before and after chemotherapy,the difference was statistically significant(P<0.05);after chemotherapy the serum VEGF and VEGFR-2 in CR+PR group and SD group were significantly lower than that in PD group,and the serum VEGF and VEGFR-2 in CR+PR group were significantly higher than those in SD group(P<0.05).The correlation analysis showed that the expression of serum VEGF and VEGFR-2 in the study group was positively correlated before and after chemotherapy(P<0.05).[Conclusion] The VEGF,VEGFR-2 were sensitive biological indicator for the occurrence and development of colorectal cancer.By detecting the levels of serum VEGF and VEGFR-2 can be helpful to evaluate clinical efficacy and prognosis of adjuvant chemotherapy.
