The regulation of MMP-9 and TIMP-1 in the process of tumor cell infiltration and metastasis of gastric cancer

摘要: [目的]研究MMP-9、TIMP-1在胃癌肿瘤细胞浸润、转移过程中的调节作用, 以期为临床诊治、评估预后提供信息。[方法]选择2016年1月~2018年8月于我院接受诊治的274例原发性胃癌作为观察对象, 通过免疫组化法对患者的胃癌细胞MMP-9及TIMP-1表达情况进行检测, 分析MMP-9、TIMP-1和肿瘤细胞浸润、转移相关性。[结果]MMP-9、TIMP-1阳性和患者的性别及年龄、胃癌肿瘤出现部位、胃癌肿瘤大小无显著相关性, 差异无统计学意义 (P>0.05) ;胃癌肿瘤细胞浸润至固有肌层、浆膜层的患者MMP-9阳性率显著高于胃癌肿瘤细胞浸润至黏膜/黏膜下层者, 差异有统计学意义 (P<0.05) ;MMP-9阳性表达与淋巴结转移程度、TNM分期呈正相关, 差异有统计学意义 (P<0.05) ;但TIMP-1阳性表达和胃癌的浸润程度及淋巴结转移程度、TNM分期均呈负相关, 差异有统计学意义 (P<0.05) ;胃癌患者的肿瘤组织内MMP-9、TIMP-1表达呈4种类型, 而MMP-9表达高于TIMP-1表达者, 肿瘤细胞出现浆膜层浸润及淋巴结转移的概率显著超过MMP-9表达低于TIMP-1、两者表达平衡患者, 差异有统计学意义 (P<0.05) 。[结论]MMP-9的阳性表达和胃癌肿瘤细胞浸润、转移显示为正相关, 而TIMP-1的阳性表达和胃癌肿瘤细胞浸润、转移显示为负相关, 两者与胃癌肿瘤细胞浸润、转移过程中发挥的调节作用为MMP-9对肿瘤细胞转移具有促进作用, 但TIMP-1对胃癌存在独立性抑制作用, 即可认为MMP-9呈阳性表达但TIMP-1呈阴性表达情况下, 说明胃癌患者的预后欠佳。Abstract: [Objective]To investigate the regulation of MMP-9 and TIMP-1 in the process of tumor cell infiltration and metastasis of gastric cancer, in order to provide information for clinical diagnosis, treatment and prognosis.[Methods]274 cases of patients with primary gastric cancer who were treated in our hospital from January 2016 to August 2018 were selected as the subjects of this study. The expression of MMP-9 and TIMP-1 in gastric cancer cells were detected by immunohistochemistry to analyze the correlation between MMP-9, TIMP-1 and tumor cell infiltration and metastasis. [Results]There was no correlation between the gender, age, tumor location, tumor size and the positive rate of MMP-9, TIMP-1, the difference was not statistically significant (P>0.05) ; The positive rate of MMP-9 in gastric cancer cells infiltrating into the muscularis and serosal layer was significantly higher than that in gastric cancer cells, the difference was statistically significant (P<0.05) ; The positive expression of MMP-9 was positively correlated with the degree of lymph node metastasis, TNM stage, and the difference was statistically significant (P<0.05) . However, the positive expression of TIMP-1 was negatively correlated with the degree of invasion, lymph node metastasis, TNM stage of gastric cancer, and the difference was statistically significant (P<0.05) ; There are four types of MMP-9 and TIMP-1 expression in tumor tissues of patients with gastric cancer. And the expression of MMP-9 is higher than TIMP-1, the incidence of serosal invasion and lymph node metastasis in tumor cells was significantly higher than that in patients with MMP-9 expression lower than TIMP-1 expression (P<0.05) . [Conclusion]The positive expression of MMP-9 was positively correlated with tumor cell infiltration and metastasis of gastric cancer. However, the positive expression of TIMP-1 is negatively correlated with tumor cell infiltration and metastasis of gastric cancer. The regulatory role of both of them in infiltration and metastasis of gastric cancer cells is that MMP-9 promotes tumor cell metastasis. However, TIMP-1 has an independent inhibitory effect on gastric cancer. It can be considered that MMP-9 is positively expressed but TIMP-1 is negatively expressed, indicating that the prognosis of gastric cancer patients is poor.
