Relationship between neutrophil to lymphocyte ratio level and hepatitis B virus infection/HBV induced liver cirrhosis

摘要: [目的]研究乙型肝炎患者外周血中性粒细胞和淋巴细胞计数随乙型肝炎病情进展变化情况,并探讨中性粒细胞与淋巴细胞比值(NLR)与乙型肝炎及乙肝肝硬化间的关系。[方法]将健康对照组20例,乙型肝炎组25例,乙肝肝硬化组90例患者应用血液全自动分析仪进行外周血白细胞计数并计算NLR,对比分析3组间NLR水平差异。采用Child-Pugh评分系统对乙肝肝硬化组90例患者进行分级,且分析NLR水平同Child-Pugh评分相关性。[结果]①与健康对照组比较,乙型肝炎组和乙肝肝硬化组患者具有更低水平的NLR (P<0.05);②在乙肝肝硬化患者中,随着肝脏纤维化及损伤程度加重,NLR水平呈显著下降趋势;③线性分析显示,Child-Pugh评分会随NLR的升高而降低,NLR水平同乙肝肝硬化患者Child-Pugh评分呈负相关(P<0.05)。[结论]NLR水平同乙型肝炎和乙肝肝硬化病理进程密切相关,伴随乙型肝炎的进展,NLR水平显著降低;NLR是Child-Pugh评分的预测因子,其水平与乙肝肝硬化和肝功能损伤程度负相关,可辅助预测乙型肝炎患者肝功能情况,评估肝脏纤维化程度。Abstract: [Objective] To evaluate the manifestations of neutrophil to lymphocyte ratio(NLR) in HBV infection and HBV induced liver cirrhosis and to compare these patients with those of healthy controls.[Methods] Ninety patients with HBV-induced liver cirrhosis, 25 patients with HBV infection but without cirrhosis, and 20 additional healthy controls were enrolled in this study.Serum levels of neutrophil and lymphocyte cells were measured by automated blood analyzers;Child-Pugh classification was adopted to evaluate the degree of liver cirrhosis.And the relationship between NLR and Child-Pugh classification was explored.[Results] ①When compared to control group(4.35±1.54), NLR levels were significantly reduced in HBV infection(2.71±0.89) and cirrhosis(1.96±0.86) (P<0.05).②NLR level was correlated with the severity of liver damage according to Child-Pugh classification.Mean NLR levels were notably lower in patients with advanced liver cirrhosis.[Conclusion] Decreased NLR level were found to play an important role in the development of HBV infection and HBV induced liver cirrhosis.NLR level is correlated to the histological degree of fibrosis in HBV infection and HBV induced cirrhosis, and might be a useful non-invasive progression marker in patients with chronic liver disease.
