摘要: [目的]探讨中医扶正通利(FZTL)法对胰腺癌患者生存状况的影响。[方法]采用回顾性队列研究方法,选取符合入组标准胰腺癌患者46例,其中对照组26例,采用规范化疗、放疗、营养支持等西医方法综合治疗。治疗组20例,在西医治疗的基础上采用中医FZTL法治疗,随访观察2组病例生存率,中位生存期,Kaplan-Meier生存曲线,并进行Cox回归多影响因子分析。[结果]随访率为98%,随访时间2~52个月,中位随访时间8个月;治疗组6、12、18、24、36个月累计相对生存率分别为69%、43%、43%、43%、29%,而对照组分别为44%、20%、15%、8%、8%,2组整体比较,差异有统计学意义(P<0.05);总生存期治疗组为14个月,对照组为7个月,2组比较,差异有统计学意义(P<0.05);Cox回归分析结果显示:中医FZTL法与肿瘤分期是影响因子,其中前者是保护因子,而后者是危险因子,FZTL法降低死亡风险61%。[结论]中医FZTL法可以延长未经手术治疗的胰腺癌患者生存期、提高生存率、降低患者死亡风险。具有较好的临床疗效和科研价值。

Abstract: [Objective][WT5BZ]To study the effects of TCM FuzhengTongli Regimen(FZTL) therapy on the survival of patients with pancreas cancer.[WT5HZ][Methods][WT5BZ]A retrospective cohort study was employed.A total of 46 cases of pancreas cancer were enrolled,of which 20 in treatment group treated with FZTL therapy on the basis of western medicine therapy,while 26 in control group with comprehensive treatment of standardized chemotherapy,radiotherapy,nutrition support and other western medicine therapies.The follow-up observation covered survival rate,median survival time and Kaplan-Meier survival curve of the two groups before Cox regression multiple-factor analysis was carried out.[WT5HZ][Results][WT5BZ]Follow-up rate was 98%.Follow-up lasted for 2 to 52 months.Median follow-up lasted for 8 months.FZTL therapy can improve the survival rate of the observed.The accumulated relative survival rate of FZTL group after 6,12,18,24,36 months were 69%,43%,43%,43%,29% respectively,while 44%,20%,15%,8%,8% in control group.The difference between the two groups was statistically significant,with P<0.05.The therapy can prolong the total survival time of pancreas cancer,14 months in FZTL group while 7 months in control group.The comparison between the two groups showed a 7-month prolonging of median survival time and a statistical significance with P<0.05.Cox regression analysis results showed that FZTL therapy and neoplasm staging were influential factors,the former being protective and the latter being hazardous.FZTL therapy can lower the mortality risk by 61%.[WT5HZ][Conclusion][WT5BZ]TCM FZTL therapy can prolong survival time,improve survival rate and lower mortality risk of pancreas cancer patients without surgery.Therefore,it is of good clinical efficiency and scientific value.
