Investigation and analysis on characteristics of dominant intestinal flora in patients with ulcerative colitis and crohn's disease and its relationship with pathogenesis

摘要: [目的]研究溃疡性结肠炎与克罗恩病患者肠道菌群组成比例及分布状态,分析其与溃疡性结肠炎、克罗恩病发生的相关性。[方法]选取我院2016年1月~2019年1月收治的118例炎症性肠病(IBD)患者为研究对象,其中溃疡性结肠炎患者72例,克罗恩患者46例。另取同期在本院体检健康志愿者60例为对照组。收集研究对象红细胞沉降率(ESR)、C-反应蛋白(CRP)、WBC和PLT等指标数据,检测患者肠道拟杆菌(BD)、双歧杆菌(BL)、真杆菌(ES)、小梭菌(CD)、乳杆菌(LC)、消化球菌(PS)、肠杆菌(EMB)、酵母菌(SB)、葡萄球菌(SP)和肠球菌(EC)数量及阳性检出率,采用Logistic回归分析影响溃疡性结肠炎与克罗恩病发生的相关因素。[结果]溃疡性结肠炎组和克罗恩病组的ESR、CRP、WBC、PLT水平均显著高于对照组(P<0.05)。溃疡性结肠炎组患者肠道内致病菌BD、SB、EC、EMB数量、益生菌BL、LC、PS数量、中性菌CD数量显著高于对照组(P<0.05),中性菌ES水平显著低于对照组(P<0.05);克罗恩病组患者肠道内致病菌BD、SB、EC、EMB数量、益生菌BL、LC、PS数量显著高于对照组(P<0.05),中性菌CD、ES水平显著低于对照组(P<0.05)。溃疡性结肠炎组患者肠道致病菌BD、EC及益生菌BL、LC的阳性检出率显著高于对照组(P<0.05),3组致病菌EC与益生菌LC的阳性检出率比较差异有统计学意义(P<0.05)。相关性分析显示,溃疡性结肠炎组SB数量与ESR正相关(P<0.05),BL、EMB、EC数量与CRP水平正相关(P<0.05),ES数量与WBC负相关(P<0.05),EC数量与WBC正相关(P<0.05),EMB、EC数量与PLT正相关(P<0.05)。克罗恩病组EMB、EC数量与CRP水平正相关(P<0.05)。肠道内致病菌(SB、EC、EMB)菌群数量高及益生菌(BL、LC)数量低是影响溃疡性结肠炎和克罗恩病发生的危险因素(P<0.05)。[结论]溃疡性结肠炎和克罗恩病患者肠道内菌群存在失衡现象,致病菌数量增加,致病菌数量高及益生菌数量低是影响溃疡性结肠炎和克罗恩病发生的危险因素。Abstract: [Objective] To study the proportion and distribution of intestinal flora in patients with ulcerative colitis and Crohn's disease, and to analyze its correlation with ulcerative colitis and Crohn's disease.[Methods] 118 patients with inflammatory bowel disease(IBD) admitted to our hospital from January 2016 to January 2019 were selected as the study subjects, including 72 patients with ulcerative colitis and 46 patients with Crohn's disease.Another 60 healthy volunteers were selected as control group during the same period.Data of erythrocyte sedimentation rate(ESR), C-reactive protein(CRP), white blood cell count(WBC) and platelet count(PLT) were collected, the numbers and positive detection rates of Bacteroides intestinalis(BD), Bifidobacterium(BL), Eubacterium spp.(ES), Clostridium minimus(CD), Lactobacillus(LC), Peptococcus(PS), Enterobacter(EMB), saccharomycetes(SB), Staphylococcus(SP), and Enterococcus(EC) were detected, Logistic regression analysis was used to analyze the related factors of ulcerative colitis and Crohn's disease.[Results] The levels of ESR, CRP, WBC, and PLT in ulcerative colitis group and Crohn group were significantly higher than those in control group(P<0.05).The number of intestinal pathogenic bacteria BD, SB, EC, EMB, probiotic BL, LC, PS, CD of neutral bacteria in ulcerative colitis group were significantly higher than those in control group(P<0.05), and the level of ES of neutral bacteria was significantly lower than that in control group(P<0.05);the number of intestinal pathogenic bacteria BD, SB, EC, EMB, probiotic BL, LC and PS in Crohn group were significantly higher than those in control group(P<0.05), the CD and ES levels of neutral bacteria were significantly lower than those of the control group(P<0.05).The positive detection rates of intestinal pathogenic bacteria BD, EC, probiotic BL and LC in ulcerative colitis group were significantly higher than those in control group(P<0.05), the positive detection rates of pathogenic bacteria EC and probiotics LC in the three groups were significantly different(P<0.05).The correlation analysis showed that the number of SB in ulcerative colitis group was positively correlated with ESR(P<0.05), the numbers of BL, EMB, and EC were positively correlated with CRP(P<0.05), the number of ES was negatively correlated with WBC(P<0.05), the number of EC was positively correlated with WBC(P<0.05), and the numbers of EMB and EC were positively correlated with PLT(P<0.05).The numbers of EMB and EC in Crohn group were positively correlated with CRP level(P<0.05).The high numbers of pathogenic bacteria(SB, EC, EMB) and low numbers of probiotics(BL, LC) in intestinal tract were the risk factors for ulcerative colitis and Crohn's disease(P<0.05).[Conclusion] The intestinal flora of patients with ulcerative colitis and Crohn's disease is unbalanced, the number of pathogenic bacteria increases, high number of pathogenic bacteria and low number of probiotics are risk factors for ulcerative colitis and Crohn's disease.
