摘要

摘要: [目的]观察益气固肠方治疗腹泻型肠易激综合征(irritable bowel syndrome,IBS)的疗效及其对结肠组织5-羟色胺(5-hydroxytryptamine,5-HT)和肥大细胞脱颗粒的影响。[方法]选取2016年3月~2017年6月我院中医科收治的腹泻型IBS患者118例,随机分为对照组与研究组各59例。对照组给予匹维溴铵片治疗,研究组给予益气固肠方治疗,2组治疗均持续4周。观察2组治疗前后中医症候积分、肠易激综合征患者生活质量问卷(irritable bowel syndrome quality of life,IBS-QOL)评分、结肠组织5-HT、肥大细胞数量及肥大细胞脱颗粒情况和治疗中的不良反应,评价2组临床疗效。[结果]治疗后,2组中医症候积分显著降低、IBS-QOL评分显著升高(P<0.05),且研究组中医症候积分显著低于对照组、IBS-QOL评分显著高于对照组(P<0.05);治疗后,2组结肠组织5-HT水平及肥大细胞数量、肥大细胞脱颗粒比例均显著降低(P<0.05),且研究组显著低于对照组(P<0.05);2组不良反应情况比较,差异无统计学意义(P>0.05);研究组总有效率为96.61%,显著高于对照组81.36%(P<0.05)。[结论]益气固肠方治疗腹泻型IBS疗效显著,且能降低结肠组织5-HT水平,减少肥大细胞脱颗粒。

Abstract: [Objective] To investigate the efficacy of YiQiGuChang decoction and its effects on connective tissue 5-hydroxytryptamine(5-HT)and mast cell degranulation in patients with diarrhea-predominant irritable bowel syndrome(IBS).[Methods]118 patients with diarrhea-predominant irritable bowel syndrome in our hospital from March 2016 to June 2017 were randomly divided into control group and research groups,each with 59 cases.The control group was given Pinaverium Bromide Tablets,and the research group was given YiQiGuChang decoction for 4 weeks.TCM symptom scores,irritable bowel syndrome quality of life(IBS-QOL)scores,mast cell number and mast cell degranulation of 2 groups before and after treatment were observed,with adverse reactions observed.The clinical efficacy of the 2 groups was evaluated.[Results]After treatment,2 groups TCM symptom scores were significantly decreased and IBS-QOL scores were significantly higher than before(P＜0.05),while research group TCM symptom scores were lower than control group and IBS-QOL scores were higher than control group(P＜0.05);After treatment,mast cell number and mast cell degranulation proportion were significantly decreased(P＜0.05),with research group lower than control group(P＜0.05);There was no significant statistics difference between 2 group(P＞0.05);The total effective rate of research group was 96.61%,significantly higher than that of the control group 81.36%(P＜0.05).[Conclusion]Diarrhea-predominant IBS treating with YiQiGuChang decoction has significant efficacy,decrease 5-HT level in connective tissue and reduce the degranulation of mast cells.
