Analysis of heart rate variability's clinical significance of functional dyspepsia aged patients

摘要: [目的]探讨老年功能性消化不良（FD）患者心率变异率（HRV）的改变及其临床意义。[方法]选取我院2011年9月~2013年9月60例老年FD患者，32例老年消化性溃疡（PU）患者，另选38例体检健康者作为对照组，对所有患者及体检健康者行24动态心电图（HRV）检测，将各组心率变异率的时域指标与频率指标进行统计学分析。[结果]FD组SDNN、rMSSD、PNN50、HF均低于对照组，差异有统计学意义（P<0.05），SDANN、ULF、LF、LF/HF高于对照组，差异有统计学意义（P<0.05）。PU组与对照组相比，SDNN、rMSSD、PNN50、HF均高于对照组，差异有统计学意义（P<0.05），LF/HF则小于对照组，差异有统计学意义（P<0.05），SDANN、ULF、LF与对照组相比，差异无统计学意义。FD组与PU组相比较，SDNN、rMSSD、PNN50、HF均低于PU组，差异有统计学意义（P<0.05），其余指标则高于PU组，差异有统计学意义（P<0.05）。[结论]老年FD患者的心率变异性有显著地改变，具体表现为交感神经兴奋性增强，而迷走神经兴奋性下降。分析患者的心率变异性对于老年功能性患者的诊断与治疗具有很大的临床参考价值。Abstract: [Objective] To investigate the elderly patients with functional dyspepsia and heart rate variability(HRV) changes and its clinical significance.[Methods] To choose 60 cases of senile patients with functional dyspepsia and 32 cases of peptic ulcer patients in our hospital from 2011 September to 2013 September in elderly patients and another 38 cases of healthy persons as control group.and do detect for all the patients and healthy subjects for 24 dynamic electrocardiogram(HRV), then make the time domain index of heart rate variability and frequency indexes of each group statistically analyzed.[Results] In the functional dyspepsia group, SDNN, rMSSD, PNN50, HF were lower than the control group(P<0.05), with statistical difference, SDANN, ULF, LF, LF/HF higher than that of the control group(P<0.05), with significant difference.Compared with the peptic ulcer group and control group, SDNN, rMSSD, PNN50, HF were higher than those in the control group(P<0.05), with significant difference.LF/HF is less than that of the control group(P<0.05), with statistical difference.Compared with the control group, SDANN, ULF and LF do not have statistics differences.Compared with functional dyspepsia andpeptic ulcer group, SDNN, rMSSD, PNN50, HF were lower than those of peptic ulcer group(P<0.05), with statistical difference;other indexes were higher than the peptic ulcer group(P<0.05), with significant difference.[Conclusion] The aged patients with functional dyspepsia patients heart rate variability had a significant change, and the specific performance of increased sympathetic tone and vagus activity decreased.Analysis of heart rate variability in patients with great clinical value to the diagnosis and treatment of elderly patients with functional.
